20. How can I change the beneficiary of my pension contributions?

  1. Active employees, please contact your Agency’s HR Personnel or Payroll office and complete a form CO-999.
  2. Inactive, vested members of SERS, please complete a form CO-999 and submit it to: Office of the State Comptroller, Retirement Services Division, 165 Capitol Avenue, Hartford, CT 06106.  Alternatively, you may email the completed form to Osc.rsd@ct.gov or fax it to 860-702-3489. (Is fax operational?)
  3. Retired employees may change their beneficiary designation by completing a form CO-999 and submitting it to: Office of the State Comptroller, Retirement Services Division, 165 Capitol Avenue, Hartford, CT 06106.  Alternatively, you may email the completed form to Osc.rsd@ct.gov or fax it to 860-702-3489.  Please note that retired members may not change their designation of a contingent annuitant, if any.  (See question.)
  4. For participants in the Alternate Retirement Plan (ARP), the 403(b) plan, and/or the 457 plan:
    1. Visit the State’s Defined Contribution website: https://www.ctdcp.com/; or
    2. Call 844-505-SAVE (844-505-7283)*; or
    3. *Hearing Impaired: Call (TDD) 877-760-5166; or
