Inactive, vested members of SERS, please complete a form CO-999 and submit it to: Office of the State Comptroller, Retirement Services Division, 165 Capitol Avenue, Hartford, CT 06106. Alternatively, you may email the completed form to Osc.rsd@ct.gov or fax it to 860-702-3489. (Is fax operational?)
Retired employees may change their beneficiary designation by completing a form CO-999 and submitting it to: Office of the State Comptroller, Retirement Services Division, 165 Capitol Avenue, Hartford, CT 06106. Alternatively, you may email the completed form to Osc.rsd@ct.gov or fax it to 860-702-3489. Please note that retired members may not change their designation of a contingent annuitant, if any.
