Each contractor must submit a completed Campaign Contribution Certification Form as defined in Public Act No. 21-76 as an appendix to bid submissions for a state contract OR, in the case of non-competitive contract, prior to the contract execution. If there is any change in the information contained in the most recently filed certification, the contractor shall submit an updated certification no more than thirty (30) days after the effective date of such change or upon submittal of any new bid for a state contract, whichever is earlier.
