Subject: Revised Form CO-929 - Bank Account Establishment Request
  1. PURPOSE

    The purpose of this memo is to advise all state agencies that the following form has been revised:

    CO-929 Bank Account Establishment Request
  2. PROCEDURES
    The attached sample of the form may be photocopied for your agencies' use. The revised form will be in the next revision of the State Accounting Manual (SAM).
