TO THE HEADS OF ALL STATE AGENCIES

Attention: Agency Heads, Chief Administrative and Fiscal Officers, and Business Managers
Subject: Activity/ Welfare Funds Questionnaire

I. Purpose

To establish a current database of all agencies which maintain Activity and/or Welfare Funds.

II. Authority

Under the Connecticut General Statutes 4-52, 4-53, 4-54, 4-55, 4-56, 4-56a, 4-57.

III. Questionnaire

Please complete the attached questionnaire to assist our office in establishing a current database of contact personnel for Activity and Welfare Funds.
