SUBJECT: | SEBAC V - FORM FOR COST OF LIVING ADJUSTMENT ELECTION |
Comptroller's Memorandum No. 97-23, dated May 12, 1997, described a major change in the cost of living adjustment (COLA) provisions for retiring members of the State Employees Retirement System (SERS).
As referenced in that discussion Tier I and Tier II members who retire between July 1, 1997 and June 1, 1999 will be required to choose between the present and new COLA provisions.
Attached to this memorandum is a copy of the new Form CO-1100, "Election of Cost of Living Adjustment for Retirement Benefits". Commencing with retirements effective July 1, 1997, this completed form must be submitted to the Retirement & Benefit Services Division as part of the retirement application package.
Because the form will be obsolete in less than two years, it will not be available through the State's forms management program. Please reproduce the attached copy of the form for use.
