TO THE HEADS OF ALL STATE AGENCIES

ATTENTION: Personnel and Payroll Officers
SUBJECT: Schedule of Self-Administered Health Insurance Billing
Calendar Year 1998

The attached health insurance schedules for calendar year 1998 are to be used in processing self-administered billing for all health insurance vendors. All reporting to vendors including payroll maintenance must be performed in accordance with the time frames noted in the schedules.

For purposes of greater clarity, two schedules are attached.Schedule A covers payroll maintenance for current employee health insurance; Schedule B applies only to new employees indicating the initial month of coverage and the first payroll deduction based upon the month in which employment commences.

Please note the relationship of Columns 1, 4 and 5 in Schedule A. Health insurance coverage is based on a calendar month only and always begins on the first day of a month. If an employee's final paycheck covers only a partial vendor payment due in the month coverage terminates, the agency must implement one of the following procedures:

  1. On or before the applicable transaction submission date for employee maintenance, obtain a cash payment from the terminating employee to complete payment for the effective month of coverage; or

  2. Cancel the terminating employee's sub-group participation on or before the applicable transaction submission date for employee maintenance; or

  3. If the applicable maintenance date for the terminating employee has passed, obtain a refund from the vendor.

    The terminating employee's coverage through the agency sub-group will cease the last day of the month for which full payment was made to the vendor.

REMINDER: Employees must be offered COBRA continuation coverage at the time of termination (See Comptroller's Memorandum 97-58, Continuation Medical and Dental Coverage, 11/21/97).
