SUBJECT: | Long-Term Care Insurance Program Deadline for Employees Retiring August 1, 1997 |
In paychecks dated June 20, 1997 a memo was issued to all state employees regarding the Long-Term Care Insurance Program (Program). The memo advised employees considering retirement effective August 1, 1997 that they had an application deadline of July 15, 1997 to take advantage of the Program under guarantee acceptance, without medical evidence. The purpose of this memorandum is to advise that due to the overwhelming interest in the Program, together with the fact that the July 15, 1997 deadline did not provide an adequate time period for employees retiring August 1, 1997 to make necessary arrangements, the deadline has been extended.
UNUM/Worksite Benefits has extended the application receipt deadline date to July 31, 1997, provided the first quarterly premium is enclosed with the application and the employee is actively at work on July 31, 1997. Please inform retiring employees of this important deadline extension.
For more information regarding the UNUM Program benefits and the enrollment process, employees may contact Worksite Benefits at 1-800-243-3211; UNUM representatives are available to answer questions and assist employees in completing the enrollment forms.
