| SUBJECT: | Statewide Forms CO-610 (Record of Pending Receipt) |
The purpose of this memo is to advise all State agencies that the following forms currently in inventory at the agency and with Vanguard Direct may be used, but will require correction. The stock items are:
CO-610 Record of Pending Receipt-Continuous
Stock # 137-02
CO-610 Record of Pending Receipt-Snap Out
Stock # 137-01
Please hand write, in red ink, the number "55" preceding the preprinted number in the document number box on all CO-610 forms, before submitting them to the Office of the State Comptroller, Accounts Payable Division for payments or to the Accounting Services Division for revenue transfers. When the current stock of CO-610 forms is depleted, the revised form will be properly numbered with the preceding "55".
CAS II and SAAAS agencies that enter their own documents must also use the above prefix number "55" on all pending receipt documents.
