SUBJECT: | Obsolete Forms Under the Forms Management Contract with the Vanguard Group |
The purpose of this memo is to advise all State agencies that the following forms are now obsolete and will not be included in the Forms Management Contract (contract award #956-A-14-647-C), with the Vanguard Group:
CO-19A | Payroll Certificate of Expenditure |
CO-156 | Request on State Central Warehouse |
CO-17PRE | Prepayment Voucher (6938-234-04) Rev. 3/93 |
CO-855 | Report of Property Loss or Damage (wind, fire, lighting, etc) was combined with form CO-853 in June of 1994 |
CO-932 | Salary Reduction Agreement |
CO-932-2 | Salary Reduction Agreement |
CO-1000 | Employee Add/Maintenance |
CO-1001 | Deductions and Other Earnings Maintenance |
CO-1002 | Permanent Salary Splits Maintenance |
CO-1004 | Time Card Entry |
CO-929 | Bank Account Establishment Form |
CO-875 | Payroll Register |
CO-876 | Vacation Pay Action |
CO-1005 | Employee Payroll Status Report |
If the need exists to utilize the above mentioned forms then agencies should make photo copies of them.
