TO THE HEADS OF ALL STATE AGENCIES

ATTENTION: All Human Resources and Benefit Officers
SUBJECT: Retirement and Benefit Divisions Forms

To better serve employees while staying current with the ever-changing state and federal regulatory requirements, the Retirement & Benefit Services Division is in the midst of both updating and redesigning many of the forms it requires for retirement purposes. The Division anticipates that this entire process will take 8 - 12 months to complete.

The Division is undertaking this project as some of its forms have not been updated for over twenty years and need clarification. For example, CO-899 "Income Payment Election Form - Option A" was last revised in September 1982. The Division is clarifying this form to ensure that members know that if a member who chooses this option dies after retirement, the spouse designated at the time of retirement will receive monthly benefits for his/her life equal to 50% of the member's reduced monthly benefit.

As an end user of our forms, the Division welcomes your suggestions as to format or wording during this process. Additionally, the Division suggests that if any language on our older forms appears unclear or ambiguous, please review the information contained in the Summary Plan Descriptions located on our website (www.osc.state.ct.us/empret ) or call the Division at (860) 702-3480.

Lastly, the Division is aware that many departments and agencies provide retiring members with agency-drafted informational summaries which explains the various benefit payment options, the paperwork involved and the retirement process. The Division is appreciative of this work and if you so desire, offers a review of your summary by Division staff.
