SUBJECT: | Instructions for Completion of Form CO-931, "Designation of Retirement System-Tier-Plan-Beneficiary" |
By Retirement & Benefit Services Division (Division) memoranda dated September 12, 2008 and September 29, 2008 we advised agencies of two projects the Division was undertaking to update and redesign many of the forms it requires for retirement purposes and make retirement and benefits information readily accessible to all who need it.
In that connection, the Division has revised the Instructions for Completion of Form CO-931 "Designation of Retirement System-Tier-Plan-Beneficiary" last provided to agencies with the Division memorandum dated August 13, 1998. The CO-931 form itself will be updated and revised in the near future and agencies will be notified accordingly.
As you know, a properly completed CO-931 form is required for all individuals employed by the State of Connecticut with the exception of individuals employed in certain classifications. The information provided on the CO-931 form is entered to the State Employees Data Base (SEDB) and used by the Division to properly identify and track employees' retirement plan benefits. The information maintained in the SEDB is also used to provide retirement benefit information on annual employee benefit statements and in the actuarial valuation of the State Employees Retirement System.
The revised instructions are now available on the Comptroller's website at www.osc.state.ct.us/rbsd/database/instruct.htm.
