TO THE HEADS OF ALL STATE AGENCIES

Attention: Chief Administrative and Fiscal Officers, Business Managers, and Personnel and Payroll Officers
Subject: Request for Information Regarding State Employees in the Reserves of the Armed Forces of the United States

In the event of activation of employees in the military reserves and National Guard, it will be necessary to supply our Office with the following information. If you have not submitted a completed form for a employee in the military reserve or in the National Guard, please do so. The attached form is provided for your use.

  1. Name of the employee;
  2. Employee number;
  3. Social Security Number;
  4. Home address and telephone number;
  5. Name, address and telephone number of any contact person for the employee;
  6. Health Insurance coverage;
  7. Branch of service;
  8. Employing agency and contact person in that agency.
