Subject:	Forms Approval Process

PURPOSE

The purpose of this memo is to remind all State agencies that there is an approval process for designing and changing your own forms to mirror Comptroller forms. Any changes to Comptroller forms that have not been approved by this Office will not be accepted.

PROCEDURES

Please send requests for design changes to all Comptroller forms to:
