SUBJECT:	Obsolete Statewide Form

I. PURPOSE

The purpose of this memo is to advise all State agencies that the following Comptroller form under the Forms Management Contract with Vanguard Direct is now obsolete:
: CO-825 Alternate Retirement Program Remittance Report (Rev. 4/77) - Stock #169-01
: Agencies should discard all CO-825 forms that they may have in stock.
