I. PURPOSE

The purpose of this memo is to advise all State agencies that the following Comptroller form under the Forms Management Contract with Vanguard Direct is no longer available to order:

 

CO-790S Vendor Check Cancellation Statement (Rev. 3/98, Stock# 123-03)

II. PROCEDURES

An updated version of the form is now available in Microsoft Excel 97 format at the Office of the State Comptroller's Web Site at www.osc.state.ct.us. The form can now be completed on-line, but must be hand signed since electronic signatures are not being accepted at this time. The reverse side of this memo contains a sample of the updated form. For those agencies that do not have internet access, you may contact the Office of the State Comptroller, at the phone number listed below to obtain forms.
